Companions/Daily Companions
Essential Functions

Personal Companion (Including but not limited to: Bed baths, help using the restroom, getting in and out of the shower, AM and PM care, etc.)

Performs various homemaking activities such as: vacuuming, dusting, sweeping, doing dishes, changing bed linens, cleaning bathrooms, and doing laundry.

Essential Functions
-
Provide health care services in patients' residences
-
Perform domestic and household tasks
-
Transport and accompany patients to doctor's offices or appointments

Administer simple prescribed medications

Assist with client's personal care activities
-
Monitor patients (vital signs, temperature, respiration, etc) and report on their condition
-
Maintain patients' care records and document provided services
-
Assist patients with mobility and physical therapies/exercises

Instruct and counsel patients and families on diet and exercise

Collect routine specimens
-
Provide companionship and basic emotional or psychological support
Skilled Nurses
-
Remaining up to date with federal and state billing guidelines.

​Have responsibility for providing support and assistance to sub-acute patients and long term care residents, which may include those with Alzheimer's and Dementia

Perform basic nursing care functions and other duties under the direction and supervision of a Licensed Nurse (RN or LPN)

Perform duties to meet the personal care needs of residents

Perform limited nursing techniques

Observe and report resident conditions, and services residents in a manner consistent with the Resident Bill of Rights

Negotiates appropriate level of care within contract terms with the payor case manager

Communicates information to care team and coordinates patient's smooth transition to the next level of care.

Obtains accurate information from physicians, patient, and payor source regarding the expected discharge plan and communicates this information to the interdisciplinary team.

Develops referrals from hospitals' social service and discharge planning department
